Selection of IT Needs Assessment Questionnaires, Coding frames and Reports from Questionnaires

This page provides a number of examples of IT Needs Assessment and evaluation Questionnaires I have developed, or helped develop over the years. Where possible coding frames are also included to make them more useful.

If you use, or develop, any of these questionnaires I would be most grateful if you could let me know.

Most of the questionnaires below use a generic introduction. I have provided this in both Word and pdf format for anyone who wants to use/develop it.

Most of the questionnaires are in pdf format. These pdf versions were derived from Teleform versions of the questionnaires (Teleform is a piece of software which allows you to create, scan and manage questionnaires). To help others develop questionnaires which appear as professional looking as the pdf /Teleform versions I have created a Word version which looks almost identical. The word version is of a draft Needs Assessment Questionnaire for a course I develop in Clinical Information Management which you can find below.

I also played a major role in the develop a two part questionnaire that was used for a UK national survey of GPs needs in 1996
